My Cat has suddenly started to poop in house

I have a 7 year old rescue cat. We have had her for 6 years and suddenly recently she has started pooing in the house on the carpet - twice in the same place in the lounge and once in the dining room. She has a cat flap, has access to it 24/7 and uses it regularly. She doesn't go out much and never has really particularly in bad weather (snow and rain over the past couple of months. She has always been very clean. Why would this start happening and how can we stop her?

there are two possibilities here either she is ill in some way, a tummy upset etc or this is purely behavioral in which case it is known as Middening. I feel that it is Middening as she has done it in the same place,
