she has a bald spot on her rear along with an infection of some kind right there. she now has bald spots behind her ears and her fur is very matted. where she has these spots she twitches. sometimes she even growl and hisses at the spot on her rear. what could this be?

Sounds to me like it is either a skin allergy or a parasite problem. Both could produce these symptoms. You really do need to get the cats skin looked at by a vet as they are the only ones who will be able to positively identify the problem and prescribe the right medication.

Your cat must be in some considerable distress as skin conditions like this can be painful and incredibly itchy and distressing. Hopefully the vet can prescribe something that will work quickly.
